Frame the problem before naming a cause

The guide separates observation from interpretation and records the affected product, batch, location, period and known impact before the analysis narrows.

Distinguish containment from correction

Immediate measures limit further impact. Permanent measures address the confirmed occurrence cause and detection gap with explicit owners, dates and expected effects.

Verify before rollout and validate in operation

D5 verifies that the selected measure addresses the confirmed cause before rollout. D6 implements it and validates effectiveness with three defined samples plus a time-bounded recurrence check.

Is an 8D report simply a completed form?

No. A useful report connects facts, causes, measures and evidence. Completing every heading does not prove that the underlying analysis or action is effective.

Does every case require containment?

No. Immediate containment is appropriate when an ongoing impact must be limited. If no acute impact remains, the decision and rationale can lead directly to root-cause work.

Does Doconio create an 8D case automatically from customer feedback?

No. A critical signal can support an internal decision to open a separate case. The handoff remains deliberate and traceable.

Can suppliers contribute without a Doconio account?

Yes. An addressed supplier can answer the versioned D1-D8 request through a protected external link. Internal reviewers decide which answers are accepted.
